A checklist used to review and personalise clinic policy documents

How to personalise clinic policy templates before using them

An editable policy template is useful only when the finished document reflects what your clinic actually does. Replacing the business name is the beginning of that work, not the final step. Here is a practical approach to organising the review.

1. Start with the document checklist

Unzip your pack, keep an unchanged master copy and create a working folder. Read the completion guide first. Record which documents apply to your clinic, who will review each one and what information is missing.

2. Complete your clinic details

Check the body text, headers, footers, tables and contact sections. Complete the organisation name, address and named-role fields throughout. Confirm your legal provider name, trading name, locations and responsibilities are consistent across the set.

3. Match the content to your service

Review the activities, treatments, age groups and consultation methods described. Change or remove statements that do not match your service. Describe your actual systems, equipment, escalation arrangements and working relationships, rather than leaving a proposed process that is not in place.

4. Complete clinical and operational fields

Highlight unanswered questions and assign an owner. Clinical protocols and treatment-specific details need review by the appropriate qualified professional. Avoid approving a policy with missing contacts, unconfirmed arrangements or unresolved clinical information.

5. Review, approve and put it into practice

Add the correct owner, version, approval and review details. Make the current version accessible to staff and record any training or acknowledgement needed. Use forms and registers alongside the policies so the written process can be followed in daily practice.

6. Revisit the source guidance

Check the current official requirements for your own service before preparing an application. Our suggested filing and review method is an organisational aid, not a replacement for CQC’s instructions or service-specific professional advice.

Choose a starting point

Our Essential and Complete packs contain editable Word documents with different levels of operational and treatment-specific detail. Both need personalisation and review; application support is agreed separately.
